At our organization, we're seeking an exceptional Community Care Lead to spearhead opportunities for career progression and support our team of Personal Carers (PCWs) in the community.

This integral position is open to current team members and offers a permanent part-time or full-time opportunity with a flexible roster over five days.

In this role, you will play a pivotal part in enabling our clients to live their best possible life by supporting our Community teams. Every day is unique, and tasks may involve:

  • Supervising, mentoring, and training a team of PCWs and Client Support Assistants (CSAs) to optimize support for our clients
  • Providing ongoing support to our team, including covering roster shortfalls as needed
  • Liaising with our community clients to monitor their service and meet care plan requirements
  • Pro-active participation and communication with the Community Health Professionals (HCP) team to optimize service delivery

To be considered for this important and rewarding work, you will have:

  • A minimum of two years' paid experience as a PCW or Enrolled Nurse (EN) working in the community and be a current team member
  • An empathetic, patient, and caring nature
  • A motivation to support junior colleagues
  • Flexibility to prioritize client requirements
  • Availability to work a minimum of 25 hours per week, Monday – Friday roster
  • Exceptional verbal and written communication skills
