Care Manager Position

We are seeking an experienced Care Professional to join our team in the Sunshine Coast area. As a key member of our care coordination team, you will play a vital role in delivering high-quality care and support across the region.

  • Conduct thorough assessments of clients' needs and develop personalized care plans that cater to their unique requirements.
  • Manage client budgets effectively, ensuring efficient use of resources while maintaining exceptional care standards.
  • Collaborate closely with healthcare professionals and other stakeholders to deliver comprehensive care services.
  • Monitor and review client progress regularly, making adjustments to care plans as needed to ensure optimal outcomes.

To be successful in this position, you will need:

  • A tertiary qualification in Nursing, Allied Health, Social Work, Welfare, or Human Services.
  • Previous experience working in Aged Care and/or Disability services, preferably as a Care Manager or Case Manager.
  • Current National Police Clearance, Working with Children/Blue Card, and Disability Services Positive Exemption Notice Card (or willingness to obtain).
  • A valid Driver's License and reliable vehicle.
  • Experience in Home Care Packages or relevant programs is required.
  • Demonstrated ability to manage conflicting priorities, excellent time management skills, and autonomy.
