Job Summary

This role supports the professional growth and career development of care coordinators within a multidisciplinary team that provides care for children, young people, families, and carers in the community.

Key Responsibilities
  • Develop and implement comprehensive care plans that address the health needs of children aged 0–5 years and their families.
  • Collaborate with families, carers, and the multidisciplinary team to plan, implement, and evaluate care services.
  • Deliver family-centered care that is safe, evidence-based, and consistent with Primary Health Care principles.
Requirements
  • Must be of Aboriginal and/or Torres Strait Islander descent and demonstrate ability to effectively engage with Aboriginal and Torres Strait Islander clients and communities.
  • Current registration as a Registered Nurse Division 1 with AHPRA.
  • Demonstrated recent experience or completion of a new graduate RN program, with experience in care for children aged 0–5 years and families/carers.
  • Experience working within a multidisciplinary team to facilitate care planning for families with complex needs.
  • Strong organizational, prioritization, teamwork, and autonomous work capabilities, with effective written and verbal communication skills.
  • Commitment to professional development, evidence-based practice, and awareness of changes in healthcare.
  • Knowledge of and ability to use information technology systems and willingness to undertake training in electronic health record systems.
  • Current NSW State driver's licence and willingness to use for work purposes.
