Job Title: Employment Transition Specialist

We are seeking a dedicated and passionate Employment Transition Specialist to support individuals living with mental health conditions in achieving their employment goals.

Job Summary:

As an Employment Transition Specialist, you will be responsible for managing a caseload of participants with mental health disability and providing tailored support to help them secure meaningful employment.

Key Responsibilities:
  • You will deliver services using the Individual Placement & Support (IPS) Model, which focuses on empowering participants to take control of their employment journey.
  • Support participants through every step of their journey, including vocational profiling, job search, resume and interview preparation, placement, post-placement support, and ongoing support.
  • Strive to exceed individual and team performance targets, delivering sustainable employment outcomes that make a positive impact on the lives of our participants.
  • Build strong partnerships with local employers to create inclusive job opportunities that cater to the diverse needs of our participants.
  • Work collaboratively with NDIS and community providers to ensure wrap-around supports for participants' wellbeing and overall success.
  • Maintain accurate records and contribute to continuous improvement in service delivery by identifying best practices and areas for growth.
Requirements:
  • A relevant Certificate III or higher qualification in a field related to employment services, mental health support, or disability services.
  • Experience in employment services, mental health support, or disability services, with a proven track record of delivering successful outcomes.
  • Strong communication skills and ability to build trust with participants, their supports, and employers, ensuring effective collaboration and support.
  • Business development skills and local employer connections, with the ability to identify and pursue new business opportunities that align with our organization's goals.
  • A current driver's licence and vehicle, with the ability to travel to various locations as required.
  • The ability to obtain all required clearances and maintain confidentiality in accordance with organizational policies and procedures.
