Signature Care Operations is seeking an experienced Clinical Care Coordinator to join our team at our 190-bed aged care residence in Wagga Wagga. As a Clinical Care Coordinator, you will play a key role in providing clinical leadership and mentoring to our team of Registered Nurses, Enrolled Nurses, and Personal Care Assistants.
Key Responsibilities:
- Provide clinical leadership and guidance to the care team to ensure high-quality care is delivered to residents.
- Drive clinical outcomes and maintain a high level of care through effective care planning and coordination.
- Collaborate with the Clinical Managers and Residential Manager to develop and implement care strategies that meet the needs of residents.
- Supervise and develop the skills of the care team to ensure they have the necessary knowledge and skills to provide high-quality care.
- Contribute to the development and implementation of policies and procedures that promote high-quality care and resident safety.
Requirements:
- At least 2+ years of experience in a Clinical Care Coordinator or similar role in the aged care or acute sector.
- Current unrestricted registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Excellent clinical knowledge and assessment skills, with the ability to apply this knowledge in a fast-paced environment.
- Experience in staff supervision and development, with a focus on building the skills and confidence of the care team.
- Strong communication and negotiation skills, with the ability to work effectively with residents, families, and other healthcare professionals.
- A working knowledge of funding and accreditation processes, with the ability to apply this knowledge in a way that promotes high-quality care and resident safety.
- Knowledge of care planning principles and practices, with the ability to develop and implement effective care plans that meet the needs of residents.
