Job Opportunity:

We are seeking a highly skilled and compassionate Dedicated Home Healthcare Professional to provide high-quality home care services to our private duty clients in Melbourne, AU.

Key Responsibilities:

  • Conduct initial home visits to assess patient needs and develop personalized care plans
  • Continuously monitor patient progress and revise or reevaluate care plans as necessary
  • Document daily clinical notes and progress updates on the patient's clinical record
  • Communicate effectively with patients' physicians and other healthcare professionals to ensure delivery of appropriate patient care
  • Provide skilled nursing care as needed to support patients' optimal health and well-being

Requirements:

  • RN/LPN with at least 1 year of experience preferred; new graduates welcomed
  • 1 year of Home Care experience preferred but not required
