We are seeking a full time and part time Occupational Therapist to join our dynamic team in the community. As a Community-Based Occupational Therapy Professional, you will provide evidence-based clinical care in the community with a focus on Home Care Package, Commonwealth Home Support Program, and Hospital Substitution programs.

Key Responsibilities

  • Deliver targeted Occupational Therapy for clients with complex health conditions or age-related care needs
  • Prescribe aids, equipment, and home modifications to support client goals
  • Collaborate in a multidisciplinary team for optimal client outcomes
  • Engage in our clinical supervision program with a dedicated supervisor
