We are seeking a skilled healthcare professional to fill this part-time position in the Acute Care of the Elderly (ACE) model, located at a major hospital.
- The ACE model provides 24/7 general medical care for older persons with acute care needs and geriatric syndromes.
- You will work as part of a multidisciplinary team, including medical, nursing, and allied health professionals, as well as geriatric consultant care.
The ideal candidate will have experience in aged care and be able to demonstrate a patient-centred approach to care. You will need to be able to work independently and as part of a team, and be able to communicate effectively with patients, carers and colleagues.
Key Responsibilities- To provide high-quality care to patients in the ACE unit.
- To collaborate with the multidisciplinary team to develop and implement individualised care plans for patients.
- To maintain accurate and up-to-date records of patient care.
- To participate in education and training opportunities to enhance your skills and knowledge.
- Current registration with the relevant governing body.
- Current and satisfactory police check.
- Current and satisfactory working with children check.
