We seek individuals who can build strong connections with clients and their families, assign staff to clients based on qualifications and availability, organize support for clients, monitor and report on their progress, refer clients to agencies that can provide additional help, assess client needs and resources for health, welfare, and other facilities and services, respond promptly to emails, inquiries, and requests from clients, staff, and management, maintain accurate records of incidents and feedback as required, re-organize work priorities to meet the needs of a fast-paced environment, support all clients to meet their goals, conduct Client Care Assessments and Care Planning, provide clinical guidance to other team members for client management, discuss and promote our services with medical professionals and referral sources.
