We are seeking a Case Manager to join our team in delivering counselling, support, and therapy services. This role will support individuals with mild to moderate mental illness, focusing on those in hard-to-reach areas and at risk of suicide.
Key Responsibilities- Refer clients to access specialist services and support groups.
- Evaluate interventions and modify safety, support, and recovery plans where necessary.
- Provide information and education to clients, carers, families, and service providers through individual and group facilitation.
- Offer outreach services across the region, preparing for and participating in case work, focus groups, community events, and partnerships.
- Provide or arrange support for families and carers as appropriate.
- Promote mental health services and resources for client wellbeing.
- Minimum Diploma level in Mental Health, Community Services, Counselling, or Social Work.
- Bachelor's degree in Counselling or Psychology or equivalent.
- 1–2 years' experience in a mental health-related service.
- Demonstrated knowledge of case management for vulnerable, disadvantaged, and complex needs clients.
- Experience and/or understanding of working with remote Aboriginal communities and their unique needs.
- Understanding of working within a stepped care approach to mental health.
- Ability to communicate and network with stakeholders.
- Computer literacy with multiple database proficiency.
- National Police Certificate.
- WA Driver's Licence.
- Valid Working with Children Check or ability to obtain one.
- Regional zone allowance included in annual salary.
