Our Home Care and Disability Team is seeking a dedicated professional to join us as a Support Services Officer. This full-time, permanent position involves supporting and mentoring a team of support workers to deliver exceptional person-centered care to clients within the community.

Job Description:

We are looking for an individual with extensive experience and knowledge of Home Care and Disability requirements. The ideal candidate will have a strong ability to lead and motivate a team, excellent interpersonal communication skills, negotiation skills, and written communication skills.

The successful applicant will be required to conduct risk assessments, manage client relationships, and provide on-call support when needed. They will also be responsible for building and maintaining client relationships in collaboration with support workers.

Responsibilities:

  1. Monitor and manage risk to vulnerable clients living in the community
  2. Lead a team to meet full compliance with company policies and procedures including Work Health and Safety
  3. Conduct required documentation including assessments and care plans
  4. Act as a liaison between clients and support workers
  5. Perform on call duties when required and attend to support services as required to ensure continuity of care

Requirements:

  • Extensive experience and knowledge of Home Care and Disability requirements
  • Experience in leading and motivating a team
  • Excellent interpersonal communication skills, negotiation skills, and written communication skills
  • Evidence of Influenza and Covid-19 Vaccinations
  • Working with Children Check (or willing to get)
  • Valid Driver Licence
  • NDIS Worker Screening Check
  • Strong computer skills; keen to attend training and learn new programs
