Job Title:

Care Coordinator


About the Role:

We are seeking a dedicated and experienced Care Coordinator to support vulnerable young people in need of assistance.


Key Responsibilities:
  • Deliver high-quality, person-centred care in a therapeutic environment
  • Coordinate care plans, facilitate meetings, and liaise with support agencies to contribute directly to shaping a brighter future for children in care
  • Supervise a team of youth worker professionals, including facilitation of team meetings and performance management as needed

Required Skills and Qualifications:
  • Minimum 2 years' experience working with children with challenging behaviours in a residential setting or in care
  • Ability to work flexible hours and accommodate roster changes
  • Ability to obtain relevant certifications and training
