Patient Care Professional

To provide holistic care to patients, incorporating the nursing profession's values of knowledge, theory and practice.
The ideal candidate will have a strong background in nursing, with experience in patient care coordination. They will be able to assess, plan, implement and evaluate the needs of patients and their families, working collaboratively with interdisciplinary teams to achieve optimal patient outcomes.
Required Skills and Qualifications:- Active registration with the Nurses Association of New Brunswick;
- Diploma in Nursing or Bachelor Degree in Nursing;
- Minimum two years of specialized registered nursing experience with preference in family practice, surgical, palliative care or oncology;
- Knowledge of the target population of Extra-Mural Services and available community services;
- Valid New Brunswick Class 5 Driver's License and good driving record (proof required);
- Current CPR Level C certification issued within the last 12 months;
- Ability to work efficiently as a team member, possess leadership and coaching skills, strong interpersonal skills and good judgment and decision-making skills;
- Ability to work all shifts as assigned, maintaining a good attendance record;
- High regard for confidentiality;
- Excellent work habits and professional appearance.
