Job Title: Community Physiotherapist

Deliver person-centered care and support to seniors in their homes through our community-based programs.

As a physiotherapist, you will be working with a multidisciplinary team to provide comprehensive assessments, develop personalized care plans, and implement effective strategies to meet each client's unique needs.

The ideal candidate will have excellent interpersonal and communication skills, the ability to relate to elderly customers with empathy, and a proven ability to assess, plan, implement, and evaluate quality care.

You will also need to design both individual and group exercise programs to keep clients motivated and engaged to optimize their function and independence, as well as communicate effectively with clients, families, and caregivers to achieve seamless care coordination and accurate progress tracking.

Key Responsibilities:

  • Conduct comprehensive assessments and develop personalized care plans for clients
  • Design and implement individual and group exercise programs to promote physical function and independence
  • Communicate effectively with clients, families, and caregivers to ensure seamless care coordination and accurate progress tracking
  • Work closely with the multidisciplinary team and Therapy Assistants to achieve desired outcomes for clients

Requirements:

  • Current FULL registration with AHPRA as a Physiotherapist (essential)
  • Excellent interpersonal and communication skills
  • Ability to relate to elderly customers with empathy
  • Proven ability to assess, plan, implement, and evaluate quality care
  • Current Australian driver's licence and reliable vehicle (essential)
