Job Description:

  • Clinical delivery and supervision of clinical and non-clinical team members in a community setting
  • Intake processing, clinical care planning and reporting
  • Conducting client care assessments
  • Supervision and management of care staff
  • Scheduling appointments and in-home services to appropriate staff based on discipline, availability and geographical requirements
  • Coordinating the provision of quality person-centred care according to the client's needs, service/care plans
  • Clinical in-home services include care plan assessment, medication, incontinence, chronic disease, complex wound management, palliative care, and diabetes

Responsibilities:

We are seeking an experienced, committed and qualified Registered Nurse to work alongside our clients enhancing their health/well-being and enabling them to stay in their own homes. You will need to be friendly, positive, and determined to make a difference.

Qualifications:

  • Qualified Registered Nurse
  • Current AHPRA registration
  • Australian National Police check
  • Professional Indemnity Insurance
  • Experienced in assessing and planning care needs within the community
