About the Role

We are seeking a skilled Support Worker to join our team at Chorus. As a Support Worker, you will play a vital role in supporting seniors and NDIS customers to remain in their homes. This role is based in Port Kennedy and surrounding areas.

Key Responsibilities

  • Provide tailored person-centered services and activities to customers with varying support needs.
  • Work alongside customers to identify and achieve their goals and aspirations.
  • Develop and maintain strong relationships with customers, their families, carers, and community agencies.
  • Collaborate with other professionals to ensure comprehensive support for customers.
  • Contribute to a positive and inclusive team environment.

Requirements

  • Certificate III Individual Support - Aged Care or Disability, or other relevant qualification.
  • Experience working with seniors, people on the mental health recovery journey, and those living with disability.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Current Australian Drivers Licence with no restrictions.
  • Reliable and roadworthy motor vehicle to travel to customers homes and assist them to access the community.
  • Comprehensive motor vehicle insurance and registration.
  • NDIS Worker Screening Check (or willingness to apply for one prior to commencing employment).
  • Current First Aid Certificate with 12 months or less currency (or willingness to apply).

What We Offer

  • Attractive salary packaging options (up to $15,900 per year tax-free) and an additional $2,650 for Meals and Entertainment.
  • Hourly rate of $31.04.
  • Part-time permanent position.
