Home Health Care Professional

The Care at Home Officer is a vital role in delivering quality care and support to clients in their own homes. This position plays a critical part in the aged care sector, where the successful candidate will have a Certificate III in Aged or Community Care, previous experience within the aged care industry, and a current First Aid Certificate.
Key Responsibilities:- Support clients with daily living, personal care, general house cleaning, shopping, respite, and social support as outlined in their care plans.
- Follow client support plans and report any changes to the Care at Home Coordinator.
- Assist with medication supervision, recording basic clinical observations.
- Liaise with families and health professionals to ensure all client needs are met.
- A Certificate III in Aged or Community Care.
- Previous experience within the aged care sector.
- A current First Aid Certificate.
- A full Australian Driver's Licence and comprehensive car insurance.
- The ability to work weekends.

To be considered for this position, you must have or be able to obtain a current National Police Check and be willing to undergo a Pre-Employment Functional Assessment. All applicants must be eligible to work in Australia.
