 **Job Description:**

In this role, you will be responsible for supporting the health, wellbeing and connectedness of our residents. This includes working closely with residents and their families, as well as internal and external partners, to support individual wellness goals and facilitate access to tailored services such as Home Care, health education, social activities, and aged care planning.

Your key responsibilities will include supporting residents in developing and reviewing personalised Wellbeing Plans, connecting residents with appropriate services, partnering with the Resident Social Committee to deliver monthly activity calendars and community events, and assisting new residents during their transition into village life.

**Key Skills and Qualifications:**
  • Experience in customer service or wellbeing coordination.
  • Strong administrative skills and the ability to manage multiple priorities.
  • Excellent interpersonal and communication skills.
  • Confidence working independently and as part of a team.
