This is an exciting opportunity for a talented Clinical Nurse Specialist to coordinate primary health care services within the community setting. As part of our team, you will work collaboratively with key stakeholders to collect data, provide consultation services, and act as a role model to other health professionals.
Key Responsibilities:- Coordinate primary health care services for vulnerable children and families within the community setting.
- Collaborate with key stakeholders to collect data and develop strategies to improve health outcomes.
- Provide consultation services to other health professionals and act as a role model.
To be successful in this role, you will need to possess significant clinical knowledge, experience, and leadership skills in community health nursing. Additionally, you should have advanced interpersonal and communication skills, as well as knowledge of cultural factors and social determinants, particularly relating to Aboriginal health.
