We are seeking a highly skilled and dedicated Community Physiotherapist to join our team at HealthcareLink. As a Community Physiotherapist, you will play a vital role in providing evidence-based services to individuals of all ages and abilities, working across multiple service delivery models, including telehealth, community visits, and centre-based services.
Key Responsibilities:- Assess and provide interventions for children and adults living with a range of disabilities
- Work within the NDIS to maximize clients' level of function, capacity, and social engagement
- Assist clients with assistive technology assessment, recommendations, and applications
- Timely documentation and report writing
- Manage a varied caseload
- Meet company professional expectations, including meeting billables and effective client management
- Attend meetings, supervision, and personal development as required
- Build relationships with key stakeholders, including therapists, clients, and referral sources
- Relevant tertiary qualifications in a related field
- Discipline-specific professional registration (i.e., AHPRA)
- Working with Children Check (WWCC or Blue Card)
- NDIS Worker Screening Check
- Evidence of COVID-19 vaccination (or valid exemption)
- Police Check and International Police Check (only applicable to individuals who have lived overseas)
- Driver's license and access to a reliable vehicle
