Key Responsibilities
  • Coordinating and implementing individualized care plans to meet the needs of clients
  • Developing and maintaining strong relationships with clients, families, and other stakeholders
  • Collaborating with the support team to ensure comprehensive and coordinated care
  • Monitoring and evaluating the effectiveness of care plans
Requirements

To be successful in this role, you will need:

  • A certificate-level qualification in Disability or Community Services, or equivalent relevant experience
  • A valid NSW Driver's License, Class C, with the ability to drive vehicles up to 10 kilograms
  • Intermediate skills in Microsoft Office and other common systems
