Key Responsibilities- Assessment and Planning: Conduct thorough assessments of clients' needs and develop personalized care plans to ensure their independence and well-being.
- Care Coordination: Coordinate and review care delivery to ensure seamless transitions and optimal outcomes.
- Client Support: Provide direct client care and support, enabling clients to maintain their independence and develop new skills.
- Reporting and Documentation: Accurately report and document changes in client behavior, actions, or reactions, and maintain accurate records.
- Team Collaboration: Liaise and network with other service providers and community members to ensure comprehensive care and support.
- Qualifications: Tertiary qualifications in a health-related field or relevant experience in a health-related field.
- Experience: Experience in case management and client care coordination, with a strong understanding of community care principles.
- Skills: Demonstrated sound verbal, written, and interpersonal communication skills, with the ability to work effectively with diverse stakeholders.
- Certifications: Current Police Screening Certificate, First Aid and CPR Certificate, NDIS Worker Screening Check (Yellow Card), Blue Card (Working with Children Check), and a valid Queensland driver's license.
