Aged Care Emergency Services Specialist

Emergency Department OverviewThe Emergency Department provides 24/7 care with 50 beds and treatment spaces divided into zones: assessment, paediatrics, short stay, adult acute, resuscitation (paediatrics and adult), and mental health assessment. We work closely with the adjoining bulk-bill Medical Centre to offer a choice of treatment locations appropriate to patients' clinical needs.
Aged Care Emergency Service Team (ASET)An exciting opportunity has arisen to join our Emergency Department team as a Clinical Nurse Specialist Grade 2 (CNS2) in the ASET. This part-time role requires working 24 hours per week.
Key Responsibilities:- Provide comprehensive assessments and care coordination for referred patients, mainly aged 65 years and older, presenting to the NBH ED.
- Enhance care, management, and safe discharge coordination for patients in the ED, primarily targeting those over 65 years old but not limited to this demographic.
- Collaborate with ED clinicians, the multidisciplinary team, and external agencies.
- Exercise excellent organisational and communication skills and maintain a strong clinical knowledge base.
- Identify patients at risk, plan safe discharges, and refer to community services.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Postgraduate certificate in Emergency Nursing or Aged Care Nursing (or working towards).
- Minimum 5 years' experience in Emergency Nursing.
- Demonstrated ability to deliver high-quality patient care.
- Highly developed interpersonal and communication skills.
- Commitment to ongoing personal and professional development.
- Exceptional time management and organisational skills.
