Job Description

We are seeking a Case Manager to provide individualised learning programs with high school students who have disengaged from mainstream schooling.

The successful candidate will work to provide person-centred case management support to youth aged between 15 to 21 (or 25 if they are a young parent).

You will respond to referrals of clients to the service from internal and external support services, conduct assessments of suitability for support, register and induct clients into the service, and provide ongoing case management sessions.

Your key responsibilities will include developing an agreed individual learning plan (ILP) in conjunction with an agreed case plan, including goals and outcomes, and developing strong internal and external relationships with clients, staff, and key stakeholders.

Required Skills and Qualifications
  • Certificate IV or Diploma in Youth Work or Youth Justice
  • Bachelor of Psychology or Bachelor of Social Work
  • Current Working with Children Check
  • Previous experience working in a case management approach within a education or employment focused environment
  • Previous training in providing a Child Safe Environment, Reporting Abuse & Neglect and Education in Care (Mandatory Notification)
  • Current Drivers Licence
  • First Aid Certificate
