Our internationally recognized approach to care is highly valued by our clients, and we pride ourselves on our genuine commitment to service excellence.
This role is a Part-Time position, offering flexible hours, and involves working across various suburbs in the Perth West Coast area.
Key Responsibilities:
- Deliver clinical care and supervise non-clinical team members in a community environment.
- Manage intake processes, develop clinical care plans, and maintain accurate reporting.
- Conduct thorough client care assessments with an emphasis on Consumer Directed Care.
- Coordinate and ensure the delivery of quality, person-centered care tailored to each client's needs and available resources.
- Provide clinical in-home services, including care plan assessments, medication management, wound care, chronic disease management, and palliative care.
- Handle general administrative tasks related to client care documentation in compliance with relevant regulations.
- Be available for occasional on-call duties as required.
Essential Qualifications:
- Registered Nurse with current AHPRA registration.
- Current First Aid and CPR certification.
- Australian National Police check clearance.
- Professional Indemnity Insurance.
- Experience in assessing and planning care needs in a community setting.
- Valid Driver's License and access to a reliable vehicle.
- Smart mobile phone (iPhone/Android).
- Up-to-date COVID vaccinations.
Preferred Skills:
- A strong commitment to delivering high-quality, client-focused care in a community context.
- Cert IV in Training and Assessment is highly desirable.
- Excellent communication skills.
- Able to work independently as well as collaboratively within a team.
- Strong time management and organizational abilities.
- Dedication to ongoing professional development.
