We are seeking a dedicated individual with a tertiary qualification in physiotherapy and current registration with the Australian Health Practitioner Regulation Agency (AHPRA) to join our At Home Aged Care Team at VMCH.

As a physiotherapist, you will work with our team to provide best practice services to promote independence and positive health outcomes for our clients.

Key Responsibilities

• Conduct assessments and develop care plans to meet the needs of our clients
• Provide physiotherapy services to promote independence and mobility
• Work collaboratively with our team to deliver high-quality care and support
• Develop and maintain relationships with stakeholders, including clients, families, and healthcare professionals
• Contribute to the development and implementation of care plans and programs

Requirements

• Tertiary qualification in physiotherapy with AHPRA registration
• Experience working with older adults in the community
• Ability to conduct standardized assessments and develop goal-oriented care plans
• Knowledge of complex conditions impacting older adults
• Strong relationship-building skills with stakeholders
• Proficient in report writing and documentation
• Valid driver's license, police check, and rights to work in Australia
• Knowledge of funding streams, including the Commonwealth Home Support Program (CHSP) and Home Care Packages (HCP)
