Job Summary

We are seeking a highly skilled Aged Care Professional to join our team in Portland or Hamilton, Victoria. As a key member of our Home Care team, you will play a pivotal role in delivering exceptional care to our clients.

About the Role

The successful candidate will be responsible for providing initial provision and case management of Home Care Packages for clients. This includes intake and assessment, care planning, community engagement, documentation, monitoring and review, fund management, and advocacy as required.

Key Responsibilities
  • Provide high-quality care to clients through effective case management and coordination with internal stakeholders.
  • Develop and implement personalized care plans that cater to the unique needs of each client.
  • Collaborate with the Clinical Team to ensure seamless delivery of care services.
  • Maintain accurate records and documentation to ensure compliance with industry standards.
  • Manage client funds and resources effectively to ensure optimal care outcomes.
Requirements

To be considered for this role, you must have:

  • At least 3 years of experience in aged care or home care, preferably in a similar role.
  • Sound knowledge of the Aged Care Quality Code of Conduct, Aged Care Standards, and accreditation processes.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients, families, and internal stakeholders.
  • A client-centric approach and willingness to advocate on behalf of clients.
  • Proven ability to work independently, make sound judgments, and prioritize tasks in a fast-paced environment.
