Job Title: Primary Health Care Specialist
">Description: We are seeking a highly skilled and compassionate Primary Health Care Specialist to join our team. The successful candidate will be responsible for delivering culturally appropriate and comprehensive primary health care services to Aboriginal and Torres Strait Islander people in Innisfail.
">Key Responsibilities:
- Deliver high-quality primary health care services to clients, including client screening, assessment, observation, education, and follow-up.
- Work collaboratively with the community to promote health awareness and provide educational programs.
- Develop and maintain strong relationships with local service providers and stakeholders.
- Contribute to the development and implementation of health promotion strategies.
- Participate in quality improvement initiatives to enhance patient outcomes.
Required Qualifications & Experience:
- Certificate III or IV in Aboriginal and/or Torres Strait Islander Primary Health Care and/or registration with Australian Health Practitioner Regulation Agency (AHPRA) as a Health Practitioner.
- Experience working in a primary health care setting, particularly with Aboriginal and Torres Strait Islander communities.
- Strong communication and interpersonal skills, with the ability to effectively communicate with Aboriginal and Torres Strait Islander and non-Aboriginal and Torres Strait Islander people.
