Job Opportunity: Care Coordinator

The role of a Care Coordinator involves providing support and assistance to individuals who require community transport services. This includes assessing, planning, and coordinating services and supports to enable individuals to live safely and independently in their own homes.

Key responsibilities include:

  • Undertaking person-centred assessments to identify the needs and goals of participants
  • Developing and implementing care plans that meet the needs and preferences of participants
  • Building effective relationships with participants, their families, and other stakeholders to ensure seamless coordination of services
  • Monitoring and evaluating the effectiveness of care plans and making adjustments as needed
  • Ensuring compliance with relevant legislation, policies, and procedures

Requirements:

  • First Aid and CPR Certification
  • Certificate IV or Diploma qualification in Aged Care related field (or working towards or willing to obtain)
  • Working with Vulnerable People registration
  • National Criminal History Clearance
  • Current Driver's Licence and access to a registered, comprehensively insured motor vehicle for work-related purposes
