Housing Shelter Support Specialist

This role is an opportunity to make a meaningful difference in the lives of vulnerable individuals. As a Housing/Shelter Support Worker, you will be part of a dynamic team that provides essential support services to those in need.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations. You will be responsible for identifying clients' problems, needs and risks, and developing individualized support plans to address these challenges.
Key responsibilities include:
- Client Assessment and Support Planning
- Conduct thorough assessments of clients' needs and develop personalized support plans
- Work closely with clients to identify goals and objectives, and develop strategies to achieve them
- Counselling and Case Management
- Provide one-on-one counselling sessions to clients, using evidence-based techniques to address specific issues
- Manage client cases, ensuring timely and effective delivery of support services
- Community Engagement and Partnerships
- BUILD relationships with community agencies and organizations to access resources and services for clients
- PARTICIPATE in cross-agency working groups to promote collaboration and coordination of services
- Record Keeping and Reporting
- Maintain accurate and comprehensive records of client interactions and progress
- PROVIDE regular reports to supervisors and stakeholders on client outcomes and program effectiveness
