About this role:

Programmed is seeking a skilled Registered Nurse to provide high-quality care and support to our veteran and aged clients in the Southern suburbs of Perth. As a key member of our team, you will be responsible for delivering personalized care services, promoting independence, and enhancing the overall well-being of our clients.

Key Responsibilities:

  • Deliver comprehensive care services to clients in their homes, including medication management, wound care, and mobility assistance.
  • Conduct thorough assessments to identify client needs and develop personalized care plans.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
  • Provide education and support to clients and their families on health-related topics and disease management.
  • Maintain accurate records and reports, adhering to regulatory requirements and Programmed's policies.

Requirements:

  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Experience working in community or aged care settings, with a focus on home care services.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues.
  • Strong time management and organizational skills, with the ability to prioritize tasks and manage multiple clients.
  • Reliable transportation and a valid driver's license.
