Client Care Liaison

We are seeking a dedicated and compassionate Client Care Liaison to join our team. This role offers a unique opportunity to make a positive impact on the lives of our clients and their families.
In this position, you will work closely with our Community Nurses to deliver high-quality care services that cater to the individual needs of each client. Your exceptional communication skills and empathetic nature will enable you to build strong relationships with clients and their families, ensuring they receive the best possible care in the comfort of their own homes.
This is a rewarding role that requires a proactive attitude, flexibility, and excellent problem-solving skills. You will be responsible for:
- Developing and implementing personalized care plans to meet the unique needs of each client
- Coordinating care services to ensure seamless delivery and timely completion
- Providing administrative support to the care team, including data entry, record-keeping, and reporting
- Participating in quality improvement initiatives to enhance client outcomes and service delivery
