We are seeking an experienced Registered Nurse to join our team as a Community Care Specialist. As a key member of our in-home care team, you will play a vital role in providing high-quality clinical care to our elderly customers in the comfort of their own homes.
**Key Responsibilities:**
- Deliver a range of clinical support services, including wound care, medication administration, stoma and bowel care, and catheter management.
- Conduct nursing, continence, and dementia screening assessments, and support care plans tailored to each customer's individual needs.
- Work closely with families, General Practitioners, and our team to ensure a holistic approach to our customers' well-being.
About You:
To be successful in this role, you will have:
- Full AHPRA registration.
- Availability to work a minimum of 2 days per week (Monday or Tuesday and Fridays ideally).
- First Aid and CPR certification (or willingness to obtain).
- A driver's licence with a reliable and insured vehicle.
- A genuine interest in working in the community and with the elderly.
- Strong clinical skills and ability to work autonomously.
- Compassionate, professional, kind, engaging, empathetic, and helpful qualities.
- Strong communication, interpersonal, and teamwork skills.
- Double COVID vaccination and willingness to undergo a functional assessment.
