Bene Aged Care's community services team invites experienced nurses to deliver person-centred care to older adults in their homes.
About the Role:- Deliver nursing care according to individual care plans, focusing on high-quality service and patient satisfaction.
- Conduct clinical assessments, reviews, and updates to ensure seamless care delivery.
- Provide wound care, medication management, and chronic disease support, enhancing patients' quality of life.
- Maintain accurate and timely documentation to inform future care decisions.
- Foster effective communication with clients, families, and care teams to achieve optimal outcomes.
- AHPRA registration as a Registered Nurse, ensuring professional competence and accountability.
- Experience in aged care, community nursing, or similar healthcare settings, fostering adaptability and expertise.
- Excellent clinical assessment and documentation skills, allowing for precise care planning and execution.
- Strong interpersonal and communication skills, enabling collaboration and mutual understanding within the care team.
