About the Role:
Delivering High-Quality CareWe are seeking a highly motivated and experienced Exercise Physiologist to join our dynamic Restorative Care team. As an Exercise Physiologist, you will play a critical role in delivering high-quality care to HammondCare clients in the community. This is an exciting opportunity to work with a broad caseload and be part of a multidisciplinary team.
Your Key Responsibilities:- Provision of a high-quality Exercise Physiology service to HammondCare HCP (Home Care Package) and CHSP (Commonwealth Home Support Program) clients including individual 1:1 home visits and group activities.
- Assess and provide intervention to clients in their homes, carry out in-home rehabilitation programs to improve functional independence and quality of life.
- Collaborate closely with our skilled MDT team to assess clients' functional care needs, set care goals, and provide treatment and interventions to deliver the best possible client outcomes.
- Participate as a member of the HammondCare team in the application of the Mission and values to the pursuit of its objectives in a person-centred, continuous quality improvement work environment.
- Participate in quality improvement projects and continuing professional development.
- Provide clinical supervision for students, if appropriate.
