About Our Case Management Role

We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will work with children and young people in Out of Home Care (OOHC) to achieve their highest potential.

This role involves case managing children and young people in OOHC and supporting them, their carers and birth families to achieve the best outcome. You will work in a team that is driven, friendly and committed to the role.

Key Responsibilities:

  • Work with a range of designated clients, including children, young people and their families.
  • Engage with clients using appropriate assessment and risk mitigation strategies.
  • Conduct thorough assessments to develop, implement and review client plans, ensuring active involvement of children and young people.
  • Facilitate transparent communication by openly sharing client plans with clients and relevant agencies.
  • Coordinate with various service providers to ensure comprehensive support delivery.

Required Skills and Qualifications:

  • Degree qualified in social work, early childhood, psychology or similar.
  • At least 12 months case management experience working with at risk children, young people and/or families in a child protection focused role.
  • Excellent understanding of child protection issues and client vulnerabilities.
  • Good written and verbal communication skills.
  • Current Drivers Licence.
