Job Description

The NSW Health organization seeks a skilled Transition and Physical Health Coordinator to join their team. This role is responsible for coordinating discharge planning, managing post-discharge follow-up clinics, and ensuring the physical health of consumers is monitored and interventions are put in place.

About You

To be successful in this position, applicants require qualifications eligible for membership of the Australian Association of Social Workers or qualifications recognized for registration with the Occupational Therapy Board of Australia. A registered nurse with experience working in the clinical area of their specified post-graduate qualifications or such other qualifications or relevant equivalent experience is also required. Strong communication and collaboration skills are essential for working within a multidisciplinary framework to plan, deliver, and coordinate patient care.
