Job Title:

The Community Geriatrics Team Lead position is a unique opportunity to make a meaningful impact on the lives of older adults in their own homes.

This role requires a registered nurse with at least 3 years of post-registration experience or an allied health professional holding an appropriate degree level qualification and membership with the relevant professional association. Experience in supervising students and less experienced staff, as well as effective leadership skills, are also essential for success in this position.

Key Responsibilities:

  • Provide leadership to the Community Geriatrics team, including supporting recruitment, performance management and development, rostering, allocating tasks, and other human resource management
  • Mentor and support other professional members of the team
  • Link clients and carers with relevant community resources and services
  • Communicate with the client's family, carers, and other service providers

Requirements:

  • Registered Nurse with at least 3 years full-time equivalent post-registration experience or an Allied Health professional holding an appropriate degree level qualification and membership (or eligibility for membership) with the relevant professional association, and or registration with the relevant Board
  • Experience in supervision of students, and less experienced staff
  • Effective leadership skills, including highly developed communication, problem solving, conflict resolution, and negotiation skills
  • Clinical experience as a senior practitioner within a subacute setting, including clinical experience with complex patients
