Key Responsibilities
  • Deliver clinical care and supervision to clinical and non-clinical team members in a community setting
  • Process intakes, develop care plans, and conduct client assessments
  • Supervise and manage care staff, scheduling appointments and in-home services as needed
  • Coordinate the provision of person-centred care according to client needs, service plans, and available funding
  • Provide clinical in-home services, including care plan assessments, medication management, and wound care

Requirements
