About the Role

A dynamic opportunity has emerged for a passionate Dementia professional to join our organisation on a permanent, full-time basis.

Key Responsibilities:
  • Understand and engage with our organisation's Mission in Action, Model of Care, Case Management Framework, and practice guidelines, policies, and procedures.
  • Provide dementia-specific expert advisory services within our services through participation in regular clinical review meetings and on-the-ground support across all sites.
  • Be an expert in and utilise the Model of Care, Case Management Framework, and practice guidelines, policies, and procedures are adhered to in individual client consulting work.
  • Build staff capacity through communication, education, mentoring, and coaching in practice, including spending time with the team on the ground, conducting case study discussions, and reviewing documentations.
  • Plan engagement with individuals and teams to achieve agreed practice development objectives and quality improvements.
  • Support through mentoring and modelling in onsite activities and be an active part of CRT and CAC.
  • Access current evidence-based guidance and knowledge, ensuring it is aligned with our organisation and utilise to inform all activities in the role.
  • Utilise agreed processes to communicate, monitor, and manage work delegations and projects, including evaluation and reporting.
Requirements:
  • A minimum of 3 years' experience in the Australian Aged Care Industry and able to demonstrate currency in your field of expertise.
  • Registered Nurse or Allied Health qualification, with further education in Certificate IV in Training and Assessment, including TAELLN411 or higher, is highly desirable.
  • Demonstrated skills in teaching, mentoring, and coaching others.
  • Knowledge and skills in relation to adult learners and development of learning resources.
  • High-level negotiation, communication, and relationship-building skills.
  • Ability to prioritise multiple tasks and meet deadlines.
  • Problem-solving and decision-making skills relevant to the aged and dementia care context.
  • Strong interpersonal and communications skills, both written and verbal.
  • A current driver's license and safe driving record.
  • Willingness to travel between our sites and services and across Australia.
  • Strong administrative and computer skills using Microsoft Office products such as Outlook, Word, Excel, PowerPoint, and Teams.
  • Ability to work independently and as part of a team.
