Nurse Practitioner

BaptistCare is seeking a dedicated Registered Nurse to join our team in the Canberra area. As a key member of our care team, you will be responsible for coordinating the delivery of high-quality care and services to support our customers to remain living at home.
Key Responsibilities- Develop genuine connections with our customers and loved ones, and assist them to implement innovative strategies that will optimise their health and well-being to support independence.
- Coordinate the delivery of care and services that meet the individual needs of our customers, including medication management, wound care, and mobility assistance.
- Work collaboratively with our multidisciplinary team to ensure seamless care coordination and effective communication with customers, families, and healthcare providers.
- Contribute to the development and implementation of care plans that promote customer independence, dignity, and quality of life.
- Provide education and support to customers and their families on healthy living, disease prevention, and management of chronic conditions.
- Australian driver licence and reliable motor vehicle with a minimum Third Party Property Damage car insurance.
- Registered Nurse qualifications with current AHPRA registration.
- Proven experience working with budget and service delivery parameters.
- Demonstrated ability to lead and work as part of a team.
