Senior Care Coordinator

Job Overview:
As a Senior Care Coordinator, you will lead a dedicated care team in delivering high-quality, resident-centered care to older Australians. Your primary responsibility will be to ensure the safe and efficient delivery of care, promoting dignity and choice for all residents.
Key Responsibilities:
- Create and implement personalized care plans that cater to individual needs and preferences.
- Coordinate clinical governance and oversee resident assessments to guarantee accurate and timely decision-making.
- Ensure compliance with regulatory requirements and Aged Care Quality Standards to maintain excellence in care delivery.
- Drive continuous improvement in quality, safety, and resident experience through data analysis and strategic planning.
Requirements:
- Active registration with AHPRA as a Registered Nurse is mandatory.
- A minimum of 5 years' experience in clinical nursing, care coordination, or care management roles, with at least 1 year in aged care.
- Strong leadership and team management skills are essential for this role.
- Excellent knowledge of the Aged Care Quality Standards and clinical governance principles is required.
