Role Overview

The Lead Care Coordinator plays a vital role in the provision of person-centred care to our residents. Reporting to the Clinical Care Coordinator and Registered Nurse, this position is responsible for supervising and coordinating care to meet the individual needs, wishes, and goals of each resident.

About You

We are seeking an experienced Endorsed Enrolled Nurse or Personal Care Worker with relevant qualifications, including a Diploma of Enrolled Nursing or Certificate IV in Ageing Support. You will have a strong commitment to delivering holistic, person-centred care and possess excellent communication skills.
