A unique opportunity to contribute to the development of a dementia centre initiative, supporting individuals transitioning from hospital settings back to their homes or into residential care. This Fixed Term Contract until July 2028 is based out of Hervey Bay and focuses on implementing practical, innovative strategies to prevent, minimise or respond to changes in behaviour related to dementia. You will apply your expertise in assessment and biopsychosocial approaches to provide effective solutions.
Responsibilities- Support families, caregivers, and clinical staff across aged care and hospitals to understand how dementia impacts each individual
- Equip carers and clinicians with skills to prevent and respond to behaviour changes through modelling, coaching, and resource development, including behaviour support planning
- Develop contextualised reports that effect change on the ground and improve quality of life for people living with dementia
- Undertake assessments to understand the holistic experience of the person and their care network
- Work collaboratively with multidisciplinary teams towards best practice behaviour support
- Contribute to research, projects, and programs to meet the changing needs of the sector
- Expertise in supporting people living with dementia
- Relevant qualifications in Allied Health, Nursing, Aged Care, or Mental Health
- Experience in Australian residential aged care and community aged care
- Ability to share knowledge and facilitate change at all levels
- High-level negotiation, communication, and relationship-building skills
- Excellent problem-solving and decision-making abilities
- Capacity to prioritise tasks and meet deadlines
- Strong written and verbal communication skills
- Effective stakeholder management
- A current Australian driver's licence and willingness to travel
Estimated annual salary: $85,000 - $110,000 AUD (dependent on experience)
