Aurora Community is seeking an experienced permanent part-time Case Manager and Discharge Coordinator as well as a temporary part-time Case Manager and Discharge Coordinator.

**Job Summary:**

We are looking for a skilled and compassionate professional to join our team as a Case Manager and Discharge Coordinator. The successful candidate will be responsible for coordinating the discharge process for patients, ensuring a smooth transition from hospital to home or other care settings.

**Key Responsibilities:**

  • Develop and implement individualized discharge plans for patients
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Coordinate with community services and healthcare providers to facilitate discharge
  • Provide education and support to patients and families
  • Monitor and evaluate the effectiveness of discharge plans

**Requirements:**

  • Demonstrated knowledge of rehabilitation principles and practices
  • Ability to provide case management model of care
  • Sound understanding of patients' individual needs
  • At least 3 years clinical experience as a Registered Nurse with current AHPRA registration
  • Sound knowledge of local networks, facilities, and service providers
  • Experience working within an interdisciplinary team
  • Excellent interpersonal, written, and verbal communication skills

**Desirable Qualifications:**

  • Previous experience working with discharge planning for DVA patients
  • Motivated and energetic team player
  • Previous experience working with a broad patient case mix
  • Previous experience as a discharge planner/co-ordinator
  • Experienced working with patients and families towards discharging
  • Previous experience with health prevention and patient education
