Empowering Independence: Become a Home Support Assessor

As a Home Support Assessor, you will play a vital role in enabling older Australians to access the care they need to live independently. Through face-to-face home support assessments, you will help clients achieve their wellness goals and remain in their chosen community.

Your key responsibilities will include conducting assessments using a standardised national process, developing individualised support plans, and referring clients to relevant services. You will work closely with clients to understand their needs and preferences, ensuring that your support plans meet their unique requirements.

To succeed in this role, you will require excellent communication and interpersonal skills, as well as the ability to work autonomously and build strong relationships with clients. You will also need to be proficient in computer skills and able to navigate our assessment protocol.

The role involves conducting assessments at clients' homes, engaging with them to build trust and rapport, and providing ongoing support to help them achieve their goals.
