Dementia Advisor

We are seeking a highly skilled and compassionate Senior Dementia Care Specialist to join our team at Ozcare. This is an exciting opportunity for an experienced professional to make a meaningful difference in the lives of our clients living with dementia.

This rewarding role involves providing expert person-centered dementia advice, assessment, and care coordination to individuals living with dementia and their carers. You will be responsible for developing and implementing personalized support plans, working closely with local services and healthcare professionals to ensure seamless care delivery.

• Conduct comprehensive assessments to identify individualized care needs and develop tailored support plans.
• Provide high-quality dementia advice and guidance to clients and their families.
• Collaborate with healthcare teams and local service providers to facilitate coordinated care.
• Evaluate the effectiveness of care plans and implement adjustments as needed.
• Participate in service planning, management, and reporting activities to drive continuous improvement.

• A tertiary qualification in a health-related field or equivalent experience.
• Current Police Screening Certificate and Queensland driver's license.
• Demonstrated expertise in delivering community-based dementia services and care.
• Certificate IV in Training and Assessment or working towards this qualification.
• Current First Aid and CPR Certificates.

• An annual salary of $99,583, plus a company vehicle and comprehensive benefits package.
