Aged Care Clinical Assessor- Sunshine Coast region

About the Role:

The primary purpose of this role is to evaluate the needs of elderly clients to determine appropriate care and support services. This involves conducting home support assessments, developing personalized care plans, and coordinating with healthcare providers to ensure clients receive client-centered care within a framework of our organization's Mission, Vision, and Values.

  • Deliver comprehensive and home support assessments using the Integrated Assessment Tool (IAT).
  • Collaborate with non-clinical assessors to provide clinical attendance when required.
  • Develop and finalize Support Plans that address the holistic needs of clients.

Key Responsibilities include:

  • Assessing the needs of elderly clients to determine appropriate care and support services.
  • Conducting home support assessments and developing personalized care plans.
  • Coordinating with healthcare providers to ensure clients receive client-centered care.

Requirements:

  • Tertiary qualifications in a health-related discipline are mandatory.
  • Current AHPRA registration is essential.
  • Minimum one year case management experience in assessment and goal-oriented support planning in aged, community, health.
