Role Overview

This is a full-time position that involves overseeing the delivery of home care services to residents of various retirement communities in Adelaide. The successful candidate will be responsible for expanding home care services within the villages and into the local community.

The Role's Key Responsibilities

  • Assess client needs and develop personalized care plans
  • Deliver competent, quality client care in the home environment
  • Maintain ongoing responsibility for assigned caseloads
  • Schedule workloads for maximum efficiency
  • Prepare and coordinate care staff and rosters
  • E nsure compliance with government standards and regulations
  • Undertake comprehensive assessments of client care needs and risks
  • Visit clients outside of the villages but within the local community

Key Requirements

  • Tertiary qualifications in Nursing, Social Work or Social Sciences (or equivalent)
  • Diploma in Case Management (minimum)
  • 2+ years experience in aged care and case management
  • At least 3 years senior management experience in Aged or Disability Care
  • Ability to develop and maintain excellent working relationships and demonstrated customer service skills
  • Experience in managing teams of care workers
  • Commitment to providing exceptional customer service and a strong client focus
  • Ability to work independently and be a resilient problem solver
  • Demonstrated strong awareness of risk and solid report writing and administrative skills
  • Proficient with client management systems, Microsoft office and government reporting funding portals
  • Current Driver's license
  • First Aid Certificate
