Job Summary

  • Facilitate on-ground support and implement relationship-based strategies with clients, carers, and care staff.
  • Work collaboratively with peers to develop skills and expertise among aged care staff and carers.
  • Support implementation of recommendation plans and facilitate trial and implementation of brokerage items.

Key Responsibilities

  • Provide expert dementia support services to clients and their carers.
  • Collaborate with colleagues to enhance skills and knowledge among aged care staff and carers.
  • Implement recommended strategies and models of practice within client settings.
  • Ensure delivery of high-quality services that meet client needs and expectations.

Requirements

  • Demonstrated expertise in dementia support and aged care.
  • Strong organisational abilities, including effective time management and prioritisation.
  • Ability to work independently and as part of a team.
  • Commitment to customer service principles, including conflict resolution.
  • Current driver's licence and safe driving record.
  • Willingness to travel and work flexible hours as required.
